This form is to be used by those interested in volunteering for a coach or assistant position with DKB Soccer Association. Please provide as much detail as you can, as it applies to the coaching role you are volunteering for. Coaching and/or Soccer experience not necessary.
Coaching registrations will be collected and reviewed by the association. The association will begin contacting interested volunteers.
All new Coaches/Assistants MUST complete police screening forms and returning coaches who provided forms in 2018, will require a "Background Check Declaration". These forms are located on our website under the "Coaches" tab.
